ALL ATHLETES must have a valid/completed/updated physical on file in order to practice.
**A physical examination is good for two years from the date of the exam**
8th graders with a valid physical on file still need to complete the form each school year,
check the "alternate year" box on the bottom of page one OR complete the "Google Form"
